Who are we?
Targeted Health Outreach runs services for young people who have additional needs e.g. Learning Disabilities, autistic spectrum condition (ASC) including Aspergers, or those with physical disabilities.
They specifically work with:
- those who have no input from social work services.
- are aged 14 - 19
- have special educational needs (SEN) or a disability
- would like some support around developing their independence and improving their health wellbeing
What do we do?
Targeted Health Outreach or THO provides a range of sessions and services including small group and 1:1 support:

Referral to Targeted Health Outreach Team are for:
- Young people with disabilities or learning difficulties aged 14-19
- Who live in Hackney or the City of London
- Who are not receiving social care services or wouldn’t normally qualify for a social care service.
Our referral form is available in the download section of this page.
Both self-referrals and professional referrals are accepted.
All referrals are discussed by the team to ensure they meet the service criteria.
- What to do if you have a complaint
If you have a concern about the support you or a relative or friend is receiving you should talk to the person in charge of the support or the manager of the service.
If you prefer to discuss your concern with someone independent of your care team then you can contact our Patient Liaison and Advice Service (PALS).
The PALS team can help resolve queries and concerns quickly and if you are not happy with the outcome then they can advise you on our formal complaints process.
