Service details
If you have a disability or are finding it difficult to access or move around your home, you may be entitled to equipment to assist with day to day activities or an adaptation to your home.
The assessment:
If you or your representative contact us, we will arrange a convenient time for someone to meet with you and assess your needs.
An interpreter can be provided if needed.
What kind of help is there?
We can provide advice about suitable equipment for you or your home that will make things easier for you and meet your needs. Everyone's needs are different so the services offered to different people will also be different.
Loaned equipment
Some equipment (e.g. a raised toilet seat or bathing equipment) can be borrowed and other equipment can be collected from a local chemist with a prescription given to you after an assessment visit.
Prescriptions for equipment
Following an assessment, a prescription could be provided to you. This can be exchanged for simple items such as bathing aids and toileting aids, at any of the local approved shops.
To provide more choice and flexibility you could “upgrade” to an item that meets the same need but that you prefer. You would pay the difference in the cost of the item.
Adaptations
We can recommend what minor adaptations to your home, like grab rails, additional stair rails, concrete steps or outside galvanised rails might help.
Or we can recommendwhat major adaptations such as a lift or a ramp for wheelchair access might help and how these can be funded.
Other options
We want to help you live independently in your home, but if we, or you, feel that you would be better off moving to a more suitable home, we will discuss all the options with you.

- Referral Details
Call to arrange for an assessment, they will arrange a convenient time for someone to conduct an assessment of your needs. An interpreter can be provided if needed. They will aim to complete your assessments within 28 days of you contacting them
Following an assessment, a prescription may be provided to you which can be exchanged for simple items of equipment, such as bathing aids and toileting aids, at any of the local accredited retailers. Click here to see them.
- What to do if you have a complaint
If you have any feedback about the service you received – something you like or are not happy with; then do talk about it with the worker or manager in the service, so that they can look at how they deliver their service.
