Education, Health and Care Planning Team - EHCP Team
Who are we?
The Education, Health and Care Planning Team (EHCP Team) are responsible for the 'statutory assessments' of children and young people with special educational needs and disabilities (SEND).
Statutory EHC Needs Assessments
A statutory assessment is a detailed investigation to find out what your child's special educational needs are and what provision is needed that will help them.
EHC plans are issued for children whose additional needs are complex enough to require support from the local authority as well as the support that can be provided by their school.
If a child or young person has been assessed as needing additional support at a higher level than SEN Support then an EHC decision panel will agree on how the needs can be planned for.
Information about the support that is needed and has been planned will be recorded in a legal document called an Education, Health, and Care Plan or EHC Plan.
The EHCP Team has Plan Coordinators are assigned to 3 Areas Teams across Hackney:
- North Hackney
- Central Hackney
- South Hackney
Each Area Team has:
- An EHC Area Coordinator.
- EHC Plan Coordinators who work on individual children’s cases
Cases are allocated according to the school or education setting that the child attends.
Arrangements are made for children and young people who attend provision outside the borough.
All children and young people who are being assessed will have a named Plan Coordinator.
EHCP Team support for parents, carers, and service providers
The EHCP team can help with queries about:
- The process of applying for an Education, Health, and Care (EHC)needs assessment which may lead to an EHC Plan being agreed as necessary.
- The process for reviewing an EHC Plan for children and young people aged 0 to 25.
EHC Plans should be reviewed every year and every 6 months for children under 5.

- What to do if you have a complaint
If you have feedback or have a concern about the service you received please contact your named Plan Coordinator to discuss it.
If this does not resolve the issue a senior member of staff from the team can speak with you to try to address your concerns.
If you are still not happy with the service you can raise your concerns formally. This means someone who is not part of the service will look at your feedback or concern and see if anything can or needs to be done to help make things better.
If you need to make a formal complaint about the service you received you can use the London Borough of Hackney complaints procedure.

SEND Mediation and Tribunal
Often decision about who has met the threshold for an EHC Plan can cause parents significant concern. There are specific times in the assessment process when you can have decisions reviewed using specific routes.
Global Mediation can also provide formal mediation support which is a process that families must consider before going to SEND Tribunal.
The SEND Mediation and SEND Tribunal processes are the way to have issues about provision or named placements looked at.
You will be given information and contact details about all these services and processes when you receive the decision letter about the EHC Needs Assessment outcome or the final EHC Plan for your child or young person.
