As you become more independent we want to help you make good health and lifestyle choices about:

  • Relationships
  • Anything else you are concerned about

How can you book a 16th Birthday health check?

Contact the reception at your GP surgery to book your free 16th birthday health check appointment with one of the nursing team.

Appointments will be arranged at a time that won’t interfere with your studies.

For more information ask your GP or GP Practice Nurse.

You do not need to make a referral but you do need to contact your GP practice to book the appointment.

What to do if you have a complaint

Complaints can be made to a clinician or manager in the service or alternatively through the Patient Advice and Liaison Service (PALS).
