Who are we? 

We are a dedicated team of friendly and experienced support professionals who work with young people from Hackney who are in crisis.


What do we do? 

Our youth services

Our youth service based in Hackney provides a range of supported living options for young adults aged 17-25.

We work closely with the London Borough of Hackney to provide a supportive home for:

  • care leavers
  • young unaccompanied asylum-seeking children (UASC)
  • young people at risk of homelessness

Our schemes provide different levels of support based on the young persons needs. This could be an individual rooms with shared living spaces and 24 hour staff support. Older young people who are preparing to live independently may need a self-contained flat.


How do we help young people achieve their goals?

We understand each young person is unique and has different aspirations and support needs. Our support is matched to the individual providing them with choice and encouraging independence.

Each young person we work with will have a key worker who helps them settle into their new home.

In the first month of your tenancy they will work with you to decide and agree goals or outcomes and the steps needed to reach these. You will meet with your key worker regularly to check on and celebrate any progress in reaching goals.

If a young person joins us from another service, and already has identified goals we will build on these.

We develop outcomes in five key areas:

Community engagement

We encourage young people to understand how to maintain healthy relationships with family and friends and help them develop a support network as they work towards independence.

Education, training, and employment

We work with young people to identify opportunities for professional development and potential career paths.

Health and wellbeing

We support young people to understand their health needs and access services to support nutrition, sexual health, mental health, and substance misuse.

Maximising income

We help young people apply for benefits, gain employment skills, and gain confidence in managing their finances.

Safeguarding

We ensure our young people live in a safe environment and work with them to develop a risk assessment to help them manage risks in their lives.


Moving on from our service

We want to set our young adults up for success when they eventually move on to independent living. When the time comes, key workers hold sessions on independent living to ensure young people have the skills they need.

The young person will get support to find suitable accommodation and understand their tenancy rights.

Who is eligible?

Young people that can be referred for these services are generally expected to be living within the boundaries of Hackney and be aged between 17-25

Referral required?

Yes

Referral Details

Referrals for services usually come through the London Borough of Hackney’s Placement Management Unit (PMU).

What to do if you have a complaint

Outward has a complaints procedure available to customers after referral into the service.

If you are unhappy with the outcome of a referral you can appeal by writing to us explaining why you feel you need to appeal a decision.

We aim to give an initial reply within 5 working days giving the name of the person investigating your appeal (this will be a manager from the support provider who assessed your needs).

We aim to fully investigate your appeal within 10 working days of the initial response letter and give you a full written response.

We will also provide information about the local Supported Living - Supporting People Team who you can contact if you are still unhappy with the response to your appeal.

Costs

  • Customers will pay towards their rent
  • They also pay their utilities and a service charge
