Who are we?
The Integrated Learning Disabilities Service (ILDS) is a specialist team within Hackney's Adult Social Care services.
ILDS services are for people over 18 years of age who have learning disabilities and who live in Hackney and have a Doctor (GP) in Hackney or City of London.
They are a multi-disciplinary team meaning the team includes professionals from different Health and Social Care services.
How do we work with you?
The team works to meet the requirements of the Care Act 2014.
ILDS assess whether an adult referred to their service has an eligible Learning Disability that meets the services criteria. This will help them understand individuals needs of people that may work with and decide if services and support are needed and what these would be.
Services from the Integrated Learning Disabilities Service could include:
- Support with accessing Care Act eligibility assessments
- Specialist assessment, intervention and support
- Counselling
- Assertive outreach service
- Community resource service
- Mobility support and advice about aids and adaptations
- Support in learning new skills
- Support to communicate better with others
Who does the service work with to provide support?
Where appropriate ILDS may arrange help and support from the following teams:
- Preparing for Adulthood Team - integrated health and social care team that sits within the Integrated Learning Disabilities Service. The team works with young people who have learning disability from the age of 18 until their EHC Plan ends or until they are aged 25.
- Social Workers - social workers from adult services help you decide, plan, agree and put in place the support and care you need (care packages). They will regularly look at your 'care package' with you to make sure it is right. If there are safeguarding concerns your social worker can help. They are there to protect and improve the quality of your life.
- Psychiatry - to assess and understand if you have mental health needs that require some support.
- Psychology - offers a safe space to talk about any problems and to help make choices about your own life. It can also assess and provide or teach you strategies to help with behaviours that are not helping you or people around you.
- Primary Care Liaison Nurses - are there to help you access primary care services. They can help you with Doctors/ GP appointments and yearly Learning Disability Health Checks. They can also help you make Health Action Plans.
- Nursing - this service can help you to get health care support and help with supporting you to have a healthy lifestyle.
- Occupational Therapy - therapists from this team can work with you to develop and improve practical skills to make it easier for you to manage yourself. This might include looking after yourself, managing personal care or developing travel skills.
- Physiotherapy - therapists help you find ways to move around and to be more comfortable getting around.
- Speech and Language Therapy - therapists can help you with your communication needs, hearing, eating and drinking difficulties.
- Dietetics - specialist tube feeding, oral nutrition support to prevent malnutrition, and weight management.
Notes
ILDS has a Social Worker for adults with autism. Hackney's Autism Social Worker only works with adults with autism who do not also have a learning disability but who may have social care needs.

- Who is eligible?
Hackney residents over the age of 18 with a suspected or diagnosed learning disability.
The service determines whether an adult has a learning disability based on assessment against the eligibility criteria and then looks at what support they may be entitled to.
This will be as a result of their assessed IQ and adaptive functioning levels.

The person being referred must be aware of the referral.
Referrals to the service are accepted from:- family and carers
- GPs
- statutory and voluntary organisations
- Self-referrals are also accepted

- What to do if you have a complaint
How you feel about having used or experienced a service is important. Services often ask for feedback to help them understand how to make sure people they work with have a good experience.
If you have feedback about this service, something you liked or were not happy with, you can talk to the person who worked with you or, if you feel more comfortable, a service manager. They will try to help sort out any problems.
If you are still not happy with the service you can raise your concerns formally. This means someone who is not part of the service will look at your feedback or concern and see if anything can or needs to be done to help make things better.
If you need to make a formal complaint you can use the London Borough of Hackney complaints procedure.
- SEND Personal Budget - Notes
- Assessments from this service are free of charge. The Integrated Learning Disabilities Service can assess whether you are eligible for a Personal Budget or Direct Payment which if agreed means you could choose to buy your service directly from a service provider.

- Advice sign posting and assessments are services that are not charged for.
- Clients that have been assessed as being eligible for council funded support may have to pay contributions or community charges.
