Who we are:
Targeted Health Outreach Team run services for young people who have additional needs for example Learning Disabilities, autistic spectrum condition (including Aspergers) or those with physical disabilities.
The team works with those who do not get support from social work services.
- are aged 14 - 19
- have special educational needs (SEN) or a disability
- would like some support around developing their independence and improving their health wellbeing
What we do:
Targeted Health Outreach – School holiday schemes are for those with Special or Additional Educational Needs and or disabilities (SEND) and who want support to develop their independence and improve their health and wellbeing.
During school holidays and the Summer break, we provide a number of activities for young people. Some of the activities are provided in partnership with other organisations like the Geffrye Museum, Mouth that Roars and Access To Sports.
Activities may include:
- Sports Activities
- Art workshops
- Film making
- Media workshops (drama, art and music)
- Cinema and bowling trips
- Cooking
- Trips out of the Borough
- Travel training in Hackney and beyond
- Camping
- Personal safety
- Visits to local youth venues and services
The scheme runs from different venues across Hackney including some schools and college settings.
Although the scheme is free, a small contribution to some of the activities may be needed.
A range of activities are provided that are designed to increase confidence, levels of physical activity, independence skills and emotional health and wellbeing.
Activities may include:
- Kayaking
- Narrow boating
- Art workshops
- Film making
- Cooking and
- trips out of the Borough
