City and Hackney Assertive Outreach Service (AOS)
Service details
Who are we?
The Assertive Outreach Service (AOS) is a community-based tertiary service specifically designed for people with:
- serious and enduring psychotic illness with whom existing services have difficulty engaging and
- who present a high risk to themselves or others
What do we do?
The Assertive Outreach Service works to the patient’s agenda and in their chosen environment or where the patient feels most comfortable.
The AOS also provides specialist services on the management and care of mentally disordered offenders in the borough and those returning from higher security settings.
This is a seven-day-a-week service and operates extended hours during weekdays.
What does community-based services mean?
A system of care builds not only on the strengths of the child, young person, and family but also on the strengths of the community where they live.
Providing community-based services means having high-quality services accessible in the least restrictive setting possible.
A community-based system of care aims to see the home, school, and neighborhood of the family from a positive perspective, and to identify the natural supports in these familiar surroundings as part of a strengths-based approach.
