Who are we? 

Homerton School Nursing Service is a service for maintained (Local Authority Funded) schools, PRUs (Pupil Referral Units) and academies in Hackney and the City of London.

The school Health service allocates a School Nurse per school setting, and they deliver the Healthy Child Programme in Line with the ages 5-19 (up to 25 with SEND) framework. 


What do we do? 

Homerton School Nursing Service provides the following support to children and young people from reception class through to leaving school:

• New Entry Health Assessments
• Support for children with disabilities and/or additional health needs
• Referral to specialist services
• A prioritised safeguarding health offer for vulnerable, school age children.
• Development of Individual Healthcare Plans (IHCP) for all children with medical needs.
• A training offer for school and early years’ staff.
• Delivery of a universal, preventative early help, health support offer, eg. Drop ins, parent evenings and health fairs etc.

Our universal, preventative health support offer

This services also delivers a universal, preventative health offer that maximises the physical and mental health and wellbeing of school aged children in maintained schools. This may include health talks on issues such as:

• puberty (growing up) maintaining good hygiene
• exercise and healthy eating
• having healthy relationships with friends
• drop-in service for young people in secondary school


Our role in Safeguarding Children

We also offer a prioritised, safeguarding school health service to children of school age identified as:

• Children in Need (CIN)
• Children on Child Protection plans, and
• Children identified as ‘vulnerable’ in terms of safeguarding

The team work in partnership other appropriate services such as Social Care and schools to ensure children are kept safe.


Supporting children with medical needs, disabilities or additional health needs

The service provides a range of care for children with disabilities and/or medical condition in partnership with both special and mainstream schools. Support may include:

• Assistance for maintained schools to meet the school health needs of children with disabilities and additional needs.
• The development of Individual Healthcare Plans for all children with medical needs, attending maintained educational settings aged 5-19 years (25)
• Training for school staff and early year’s settings to increase their understanding of and confidence in supporting children with health conditions including asthma, epilepsy, anaphylaxis and diabetes. 


Onward referral to specialist services

Our school nurses can refer any child who might require more specialist support for medical assessment or to be seen by one of our specialist services.

These may include:
• Speech and language therapy
• CAMHS (Child and Adolescent Mental Health Services) the NHS services that assess and treat young people with emotional, behavioural or mental health difficulties.
• Children’s Social Care
• CHYPS (City and Hackney's Young Peoples Services) Plus
• Dietician
• Hackney Young Carers
• Off Centre –Young People's Counselling service
• Sexual Health Services
• Young Hackney Services

The service working hours are 9am - 5pm Monday to Friday Term Time only. 

The school's named school nurse is available to discuss any concerns about health provision for children in schools settings. They are able to work with you to resolve any issues.

Who is eligible?

School aged 5 to 18 year olds who attend mainstream schools or academy settings. 

School Aged children and young people aged 5 to 19 years old for the Healthy Child Programme 

Young people attending mainstream schools aged 19 to 25 if they have an Education Health Care Plan.  

Referrals to the School Health Service may be made as a self-referral from parents or by schools, hospital staff,  GPs and other health and social care professionals

Yes

SEND Personal Budget - Notes
SEND Personal Budgets are an option for children and young people using this service particularly those in Special School's with complex care needs. SEND Personal Budgets could be an option for children using this service who have medical needs and are receiving tuition because of these.
