City and Hackney Looked After Children Health Team Service

Who are we?
The Looked-after Children’s Health Team monitors and promotes the health and wellbeing of looked-after children and young people, aged 0-18, who've been placed in care by children’s social care services in Hackney and the City of London.
What support do we offer?
This team helps children and young people to mange their health.
All children and young people who become looked after have an initial health assessment, by the Paediatric team (doctors for children and young people aged 18 and under).
Children aged under five have review health assessments every six months.
Children and young people over five are reviewed once a year until they are 18.
After the first appointment at hackney Ark the looked-after children nursing team might provide follow up health assessments or reviews in the child’s home, school or college, or at a community clinic.
Some younger children and those with more complex needs are still seen by the Paediatric team at Hackney Ark.
Adoption Assessments are completed by a paediatrician who fills the role of Designated Doctor for Looked After Children (LAC) and also acts as the adoption advisor for Hackney and City of London.
